If you have completed your MAP Registration Form AND your payment has been processed by Q Music, you will have received a MAP Username and Password by email. With this information, you will be able to LOG IN to your MAP data form.

Your Password is case sensitive so if you can't remember it EXACTLY as it appears in your email notification, please highlight and copy it from your email and paste it into the Password box.

Please allow enough time to complete your MAP data form so that it is saved and submitted at least one or two days before your scheduled MAP session.

To start your MAP data form, just go here and log in. Then click on the EDIT button and off you go.
